of vegetovascular dystoniae — up to 59.5 per cent, hypertension — 18.3 per cent, rheumatism — 18 per cent, others — 4.2 per cent. Dramaticlly important scientific and medical prob lem of monitoring, assessment of health status and organization of implementation of measures on medical and sanitarian support of individuals — “liquidators” — having taken direct part in the Chornobyl NPP accident clean up — requires constant attention. Presently, information has been accumulated in the National Registry of Ukraine about health status of 119,344 liquidators (114,302 men and 5,042 women). Analysis of morbidity dynamics of this contingency tes tifies about presence of negative changes both in inten sive and extensive indices. After years of observation general morbidity per 1 000 examined persons has grown from 5.81 per cent in 1982 to 14.09 per cent in 1993 (Ukraine — 53.44), including neoplasms — from 8.55 per cent in 1988 to 22.02 per cent in 1992 (Ukraine — 7.73 per cent), malignant neoplasms — from 1.64 per cent to 7.19 per cent per 1,000 (including digestion, res piratory haemopoiesis organs, thyroid gland). Among diseases of endocrine system excess in 10- 15 times is noticed for indices of adult population in Ukraine (including thyroid hyperplasiae, thyroidites (10 times), diabetes mellitus). Morbidity among liquidators on class of blood cir culation system diseases (includung hypertension, is chemic heart disease) also exceed considerably indices peculiar for the Ukrainia population. If in 1988 invalidisation index among liquidators had been 2.6 per cent among men and 2.5 per cent among women, in 1990 this index increased to 12.4 per cent and 12.1 per cent respectively, and since 1991 (after making changes in criteria of inclusion of victims into I category) this index reached 34.8 per cent for men and 60.7 per cent for women, it can be connected both with changes in health status and impact of social and psychological factors. At the same time mortality indices confirm once again unfavourable situations with health status of liq uidators: despite — relatively young age of this contin gency (age, 25-45) mortality indices annually increase from 2.26 per cent in 1988 to 6.6 per cent in 1992 (in 1989 — 3.2 per cent, in 1990 — 3.3 per cent, in 1991 — 3.9 per cent, in 1993 — 5.9 per cent). The same tendencies, with lesser results, take place in assessments of health status of other suffered cate gories (evacuees and population residing on contami nated areas). Total number of deceased among contingencies suffered as a result of the Chornobyl catastrophe ex ceeds 90,000 persons for 1988-1993. Mostly effected is population of the elderly residing in contaminated areas, but the number of deceased among the liquidators reached already almost 4,000 persons (3 836 for 1988-1993, including 1,275 persons in 1993) and evacuees 2,510 (571 — in 1993). In its order for scientific investigations the Ukrain ian Health Ministry determines priorities for scientific issues of health status of people working in the 30-km zone. The following objectives will promote solution of problems of medical aid, scientific and medical support after assessment of consequences of the Chornobyl catastrophe and development of prognosis of health status of suffered contingencies: 1. Adoption of the State Program on Elimination of consequences of the Chornobyl NPP accident. 2. Priority in this Program, Goal Subprogram, be given to medical aid for victims as follows: — provision of measures on treatment and health care support of children, liquidators and suffered po pulation; — maintaing and improvement of the National Reg istry as the system of registration and provision of med ical aid for victims; — provision of enhanced level of medical aid on account of goal centralized measures with medicine and organization of specialized establishments. 3. The basic goals of the medical science should become: — scientific and medical support of the State Regis try of victims, as the system of health care provision by the way of development and implementation of mea sures of organizational and medical character, assess ment and prognosis of their health status for each indi vidual and the whole suffered population, development of medical and methodological principles of goal medi cal follow-up, treatment and sanitation of victims, retro spective dosimetry, improvement of a system of exper tise of impact of the Chornobyl NPP accident con sequences at health status of victims; — study of mechanisms of low radiation doses action and complex multifactoral impact of the Chor nobyl catastrophe on human health; — study of role of radioecological, social, hygienic and psychological factors in interrelate complex of impact on health status of people residing in contami nated areas including other categories of victims. Impro vement of the expertise on the impact of the Chornobyl NPP accident consequences effecting health status of population; — development and implementation of prophylactic medical measures, new methods of diagnostics, treat ment and sanitation of victims.
