UNWLA.ORG 29 WOMEN'S SEXUAL HEALTH by Ihor Magun, MD, FACP Although rarely discussed or addressed, sexual dysfunction affects nearly half of all wo m- en at some time in their lives. While it is more common in older women (women remain sexually active up to the ninth decade of life), it can affect women at any age. Sexual activit y is a normal human activity; not only is it necessary for conception , but it also helps solidify relationships, relieves stress , and i s an integral part of your well - being. In women, the sexual response in volves several phases — desire involving thoughts about sexual activity, arousal, orgasm , and resolution. The phases do not always fo llow this sequence and sometimes certain phases do not occur at all. Problems with sexual response can have a variety of causes. Sometimes sexual dysfunction can be traced to medications one takes. Many fall into this category, but some stand out more t han others. These include medications for hypertension, ca r- diac and psychiatric medications as well as ho r- monal therapies. Medical conditions that can cause sexual dysfunction (besides the obvious vaginal problems) include diabetes, thyroid issues, and pr oblems with estrogen levels. Communicating with either your internist or gynecologist about possible sexual issues stemming from these cond i- tions can provide a simple solution. If you do not discuss problems, they cannot be addressed. De sire involves thre e components: drive (a desire to be sexual), cognition (an individual ' s belief s about sex) , and motivation (the interpe r- sonal connection, which is very much affected by external factors such as the state of the relatio n- ship). Long - standing conflicts not on ly about sex but also about other thing s can significantly affect your sexua l desire. Addressing lack of desire i n- volves identifying the underl ying cause and di s- cussing the problem either with your partner , or wi th a professional, or with both of them . Some women during sexual arousal are unable to maintain adequate lubricat ion. This can occur in young women or with aging, but in both cases the treatment is identical — use of over - the - counter lubricants. Female orgasmic disorder is surprisingly more comm on in y ounger women. It is defined simply as a delay or absence of orgasm following a normal excitement phase. F actors such as having less frequent sexual activity can play a role , but there can also be underlying cultural, religious , and social causes. Tr eatment in these cases i n- cludes cognitive - behavioral approaches , including relaxation, yoga , and promoting a more positive view of oneself — both in mind and body image. Antidepressants of the SSRI (selective serotonin reuptake inhibitors) class are no torio us for cau s- ing female orgasmic disorder. Dosage reduction or c hange in the medication class can be very helpful in this case . It is worth noting that there are many women who have a completely normal sex ual life without reaching the orgasm phase every t ime . They may not achieve orgasm with each encou n- ter but are perfe ctly satisfied. The goal with se x- ual phases is not to work through them as a ma n- datory list, but to have a sexually satisfying rel a- tionship. This brief overview is meant to help you become more aware of the issues that can stifle the normal sexual response. Understanding the possible factors involved and discussing them with your physician and your partner c an improve your sexual life and have a signifi cant impact on your overall wel l - being .
