UNWLA.ORG 33 PAP TEST by Ihor Magun, MD, FACP The Papanicolaou test or smear, commo n- ly referred to simply as the Pap test, has been ut i- lized for decades to alert physicians to existing curable abnormalities. In the 1930s, cervical ca n- cer, now detected with the Pap test, killed more women each year than breast or lung cancer. T o- day, death from this type of cancer is relatively rare. Pap smears primarily look for cervical and/or vaginal cells that may be pre - cancerous or cancerous. They can also detect vaginal or uterine infections. A sample is c ollected from the cervix and the area around it. Then the specimen is placed on a slide or in a liquid and sent to a laboratory for evaluation. The HPV ( Human papilloma virus ) test is frequently done in conjunction with the Pap test. The timing of this t est is important. It should not be scheduled during a period , and it is preferable that for two days before the test you do not use a tampon, do not douche, use any creams or jelly, or engage in sexual activity. The appropriate frequency of Pap scree n- ing depends on age an d doctor recommendations. New guidelines were published this year in the Annals of Internal Medicine. Surprisingly, scree n- ing frequency has decreased under these new re c- ommendations. The screenings should begin at age 21. No longer is screening recommended three years a f- ter starting vaginal intercourse. Women aged 21 - 65 should be tested every three years. It is recommended to combine the Pap with the HPV every three to five years for women aged 30 and over. Screening is no longer r ecommended for women 65 or older who have had three or more normal Pap tests in a row and no abnormal Pap test result in the past ten years, or for women who had negative HPV test results in the past 10 years. Women with a positive HPV test and a no rmal Pap are require d to do a repeat of both tests or to receive a gene test called ' genotyping. ' The latter determines if they have the most co m- mon HPV 16 and 18 , which are known to be the cause of 70% of cervical cancers. Women who have received the va ccine against HPV should begin cervical cancer scree n- ing at age 21. A word about HPV: this virus can cause not only cancer (anal, cervical, oral, penile, vu l- var, or vaginal), but also most cases of genital warts in both men and women. The virus spreads t hrough skin to skin contact. Many individuals do not know they have the virus, so they may be spreading it unknowingly. Although I have already written on the HPV vaccine in Our Life , some changes have ta k- en place since then. Recently the vaccine has been approved to be administered to both men and women, ages 9 - 26. Previously, it was only a p- proved for females. Three doses are necessary, and for the vaccine to work best, it should be a d- ministered before any sexual activity begins. It is also more effective in the earlier age group, indu c- ing a higher antibody response. The vaccine is safe and produces no significant side ef fects. Since the vaccine has not been studied in indivi d- uals over the age of 26, no re commendations can be made. Finally, it is important to remember that the only cancer which the Pap test can detect is cervical cancer. It does not detect uterine, vaginal, vulvar, or ovarian cancer. Annual gynecological evaluations are necessary, and if you notice any changes that do not seem normal, you should di s- cuss them with your doctor. Pay close attention to your body, follow the new guidelines, and aim to be always at your best. Лист до редакції Дорогі Редакторки «Нашого життя»! Мені здається, що, в першій мірі, найкраще, коли старші люди самі заплянують диспозицію своїх вартісних культурних надбань. В кожнім разі і старшим, і тим, що занимаються тими справами, буде помічним, коли створити і опублікувати листу інституцій, музеїв, бібліотек, де можна висилати ці вартісні речі.
