The media and direct to consumer adverti s ing by pharmaceutical companies have raised public awareness about bladder control issues, which was once a subject or private concern that most people considered a somewhat embarrassing normal part of aging. One of the lessons learned from such adve r tising is that problems with the urinary bladder can affect both men and women; another is that it should not be considered a normal part of aging. Indeed, there are several reasons such problems o c cur and several ways t o address them. The best place to begin is to understand normal bladder function, which is quite simple. The bladder gets full, muscles contract, starting the flow of urine, and the bladder empties. Problems with the bladder begin when inv o luntary muscle contractions occur even though the bladder is not full. This can lead to strong sudden urges to urinate or something the medical profession defines as bladder urgency, a condition frequently associated with an increase in the number of urinary episodes ac companied by leakage or wetting acc i dents. Unaddressed, these symptoms can lead to social, psychological, and sexual problems. Fortunately, overactive bladder symptoms can be treated with medications and other non - tablet options that are extremely helpful. These non - tablet options require some understanding of what prompts the symptoms and some moderate lifestyle changes. For example, certain foods and beverages trigger bladder issues. These include caffeine, s o das, alcohol, spicy foods, citrus fruits and j uices, artificial sweeteners, as well as tomato - based foods. Limiting, or better yet, eliminating these from your normal diet can lead to improvement. Other good habits including "retraining" your routine by consciously and incrementally ad d ing more time b etween bathroom visits. Try to stretch out the time interval a little more each day. If you routinely go every hour, gradually increase the time by at least fifteen minutes. Eventually you may be able to stretch this out to longer periods. Let your brain b e in charge and practice “mind over ma t ter” — it’s not just a saying. Another approach is to improve pelvic mu s cle tone. This can include vaginal weight training, a process that requires holding small weights within the vagina by tightening the vaginal musc les. The recommended training regimen is twice daily for fifteen minutes for four to six weeks. Kegal exerci s es (strong pelvic floor muscle squeezes) are also helpful, but take more time and effort (30 to 80 squeezes daily for at least eight weeks). This e xe r cise routine is extremely bene - fi cial but does require commitment. Certain bladder issues cannot be corrected without surgical intervention. Discussions about medicinal interventions are obviously another o p tion but should always be used in conjunction with non - tablet recommendations. Above all, recognize that bladder issues can be managed. Stay positive and make certain you address your concerns, and your options, with your gynecologist or urologist. _________________________________________________ ______________________________ An Old Farmer's Advice The best sermons are lived, not preached.
