Many individuals, unable or unwilling to take or tolerate prescribed medica tions, are now turning to what is presumed to be a more "natural" treatment. Before I elaborate on the two most commonly used and talked about of these herbs, it should be emphasized that no herbal sup plement has ever been approved for any medicinal use by the medical community. Recent literature is filled with anecdotes on the wonderful effects of St. John's Wort. What ex actly is this saintly wort? It is, in fact, not the wort, but its extract — hyperforin-- that is the key compo nent responsible for its effectiveness. While it is only one of the many components of St. John's Wort, hy perforin has been shown to prevent the re-uptake of certain neurotransmitters that are known to be re sponsible for mood and emotional modulation. When these neurotransmitters are produced in excess amounts due to stress or other external factors, they affect the mental outlook. By preventing the reup take, hyperforin allows for the maintenance of a nor mal mood, self-esteem, and emotional balance. The positive effects are similar in action to commonly used prescribed anti-depressant medications. The recommended duration of treatment is from six to nine months with 300 mg three times daily on an empty stomach. Ginkgo biloba has been used by the Chinese as a medicinal herb for over 4,000 years. This herb is actually a combination of chemical components of which flavonoids and terpenes are most important. The flavonoids improve the circulation in small blood vessels. They also protect the brain cells against damage caused by various factors which prevent ade quate oxygenation; hence they may improve mental performance. The recommended dose is one tablet three times a day with meals. A minimum of four weeks is necessary for optimal effectiveness. There is obviously a place for the use of herbs, but a cautionary note is in order. The side ef fects (never listed with herbs) of St. John's Wort in clude gastrointestinal symptoms, dizziness and fa tigue. The herb can make skin photosensitive and should, like all herbal medications, never be taken with prescription medications except with the ap proval of a physician. Ginkgo biloba claims to have anti-oxidant effects and can help reduce blood clot ting. For this reason, patients should exercise caution when taking Ginkgo biloba with medications such as aspirin, warfarin (coumadin) or other blood-thinning agents.
